Myelofibrosis treatment currently includes four FDA-approved JAK inhibitors: ruxolitinib for frontline symptom and spleen control, fedratinib for first-line or post-ruxolitinib use, pacritinib for patients with severe thrombocytopenia, and momelotinib for those with moderate-to-severe anaemia. The R&D pipeline is moving towards targeted agents and combination strategies intended to improve anaemia, reduce marrow fibrosis, overcome resistance and potentially alter the disease course. These differences have made the market increasingly segmented by blood count, prior treatment and persistence rather than by broad disease diagnosis alone. Specialist familiarity, reimbursement coverage and an established specialty-pharmacy channel support its continued use in intermediate- or high-risk primary and secondary myelofibrosis. However, worsening anaemia, thrombocytopenia, intolerance and loss of response can restrict dosing or shorten treatment duration. Fedratinib provides another JAK-inhibitor option for adults with intermediate-2 or high-risk disease, including patients previously treated with ruxolitinib. It competes for patients who are unsuitable for the incumbent and those requiring another JAK-based therapy after progression or intolerance. Karyopharm’s Phase III SENTRY study evaluated selinexor with ruxolitinib in previously untreated patients. At week 24, 50% of combination-treated patients achieved at least a 35% reduction in spleen volume, compared with 28% receiving ruxolitinib alone.
